I had my 4 Runner in for 10,000 mile maintenance, which included an oil change. When I got home, I noticed an oil leak on the garage floor. I called the service department, drove back to Kari, and the leak was taken care of quickly. A screw was not tightened on the oil pan. A simple fix.
But if it was so simple, it should have been done right the first time.
